Chapter Adult Event Release Form - If you are not
a member of the Memphis HOG Chapter and you attend an event, let's
say as a guest of a member, this release needs to be signed for each
event that you attend.
Chapter Minor
Event Release Form
- Basically the same rules applies for this form as mentioned above
except this release form is specifically designed for minors and it
is two pages instead of one.
Special note....this form needs to be completed and signed by
the parent of the minor not a relative such as the
Grandparents or Aunts or Uncles.
ABC's of
Touring Form - This
is the Official Guidelines and the registration form of the HOG's
ABC's of touring program.
HOG Mileage Program - This is
the Official Guidelines and the application form for HOG's Mileage
Program.

Injury Report - Unfortunately, there has to be an official
report completed if there is an injury, and this is the one.
Safety Riders Skills Reimbursement Form - The name
says it all.
Membership Participation Signup Form
- Every time the Memphis HOG Chapter has an event,
you need to ensure that you sign up so that you can get your
points towards receiving the Participation Patch. This is the
form that we use for the events.
